Application for New Membership

Students wishing to apply for student membership may submit a completed application form along with a membership fee of $500. Registered chiropractors wishing to join the CDAHK are required to prepare the following documents:
Step 1
A completed membership application form
Step 2
A completed insurance application form (if you opt for professional indemnity coverage with the CDAHK)
Step 3
A certified true copy of your graduation certificate/diploma issued by a CCE approved Chiropractic College/University
Step 4
A completed insurance application form (if you opt for professional indemnity coverage with the CDAHK)
Step 5
A cheque payable to the “Chiropractic Doctors’ Association of Hong Kong Limited” (for Hong Kong memberships) or pay online by “PayPal”
Step 6

Please note:

  • Membership fee is non-refundable.
  • The cost of membership is inclusive of your professional indemnity coverage. The coverage is under a group policy of CDAHK. The insurance coverage is provided by QBE HongKong & Shanghai Insurance Ltd.
